Mission Statement

The mission of Student Records is to process, certify and maintain a permanent record of enrollment activity and educational credential attainment for students, ensuring the protection and confidentiality of these records as they pertain to internal and external customers.

Our maintenance and disclosure of student record information is governed per the Family Educational Rights and Privacy Act (FERPA) (PDF), opens in a new window requirements.

Common Forms Handled by Student Records

These forms are in PDF format and must be printed in order to submit the forms. Electronic signatures cannot be accepted at this time.

Transcript Requests and Graduation Petitions can be mailed to the address listed on this page or returned in person to the Cashier’s Office room 140 Kee Hall. All other forms can be mailed to the address listed on this page or returned in person to the Student Records Office room 142 Kee Hall.

Student Records is Also Responsible for:

Enrollment Verification – providing Enrollment Verification Services (Veteran’s Administration, OBES, Financial Aid Agencies, etc.)

Evaluation of Credit – coordinating the evaluation of transfer/articulation credit.

Official Transcripts – processing requests for official transcripts.

Refunds – processing refund requests.

Registration – processing student enrollment, which is also available online, and withdrawal from all credit classes. View the most recent class schedule.

Residency – processing of petitions for change in residency status.

Loan Deferments – processing of student loan deferments.

How do I Request an Official Transcript?

    1. Download the Transcript Request form (PDF), opens in a new window.
    2. Print the form.
    3. Fill out the form completely.
    4. Send the form to the Cashier’s Office with the $5.00 fee per transcript (if we mail it out for you) or $15.00 fee per transcript if you are picking it up the same day
      1. Mail: send the completed request form to North Central State College, Cashier’s Office, PO Box 698, Mansfield, OH 44901, make sure to include a check for the correct amount.
      2. Fax: send the completed request form via fax to the Cashier’s Office at 419-755-4782, then call their office with the payment via Debit/Credit card at 419-755-4722.
      3. Email: send the completed request form via email to cashier@ncstatecollege.edu, then call their office with the payment via Debit/Credit card at 419-755-4722.

    Student must physically sign the form

    NOTE: Remember, for it to be official it has to stay SEALED!

    1. Go to the Cashier’s Office to pick-up the transcript request form.
    2. Fill out the form completely.
    3. Turn in the transcript request form to the Cashier’s Office with payment: $5.00 fee per transcript (if we mail it out for you) or $15.00 fee per transcript if you are picking it up the same day
      1. If it needs to be mailed, you are done and the Cashier will send it to the Student Records Office for processing.
      2. If you need same day pick up, you will proceed to the Student Records office after payment to retrieve the transcript.

    NOTE: Remember, for it to be official it has to stay SEALED!

    1. Log into your MYNC account.
    2. Click on Students.
    3. Under Academic Profile, click on Official Transcript Request.
    4. Enter the institution’s name/address information and submit.
    5. Fill in payment information ($5.00 per transcript) and submit.
    6. The transcript will be sent out the next business day. If it is a state institution it will be sent electronically, if it is a non-state institution it will be sent by mail.
