IT Help Request Form

This form is for student login issues only. All other issues should be directed by phone to the IT Service Desk, 419-755-4734. Hours are M-F, 8am to 5pm. All phone requests will be addressed during these hours.

If you can’t log in:

  • Current Students: Make sure you are using your correct Username and Password. Check the Username and Password help document (PDF) to see instructions about your default login.
  • Transcripts for Prior Students: Transcripts are no longer obtained through NC State College’s IT department.  To request a transcript, from the top right corner of our home page click on Menu, then click on Transcripts.  You will be required to create an account using your home email address.  For further assistance contact the Student Records office at 419-755-4857.
  • The IT Service Desk can be reached at 419-755-4734 during normal business hours. The form below can be used outside of normal business hours.
