IT Help Request Form

This form is for student login issues only. All other issues should be directed by phone to the IT Service Desk, 419-755-4734. Hours are M-F, 8am to 5pm. All phone requests will be addressed during these hours.

If you can’t log in:

  • Current Students: Make sure you are using your correct Username and Password. Check the Username and Password help document (PDF) to see instructions about your default login.
  • Transcripts for Prior Students: Please call the IT Service Desk for assistance. The above username and password will not work since you are no longer attending NC State.
  • The IT Service Desk can be reached at 419-755-4734 during normal business hours. The form below can be used outside of normal business hours.
