Thrive! Daily Living Needs Survey

Being in college is not easy. It can be very stressful. You are not alone. NC State can help you connect with people who will help you fulfill daily-living needs to enhance your academic success now and in the future.

Take 5 minutes to respond to 10 short statements about your daily needs.

This study, "Daily Living Needs Survey," aims to identify students who may be at risk of not meeting their daily living needs (basic needs). Your participation is voluntary, and you can withdraw at any time without penalty by closing the survey. For reporting purposes, all data collected will be kept confidential and reported in an aggregated, anonymous format. There are no anticipated risks associated with this survey. The knowledge gained from the study may help improve services to students at the college. By continuing with this survey, you indicate that you have read this information, are at least 18 years old, and voluntarily agree to participate.

1.) Housing

I am at-risk of not having a safe, stable, or regular place to sleep and live.(Required)

2.) Utilities

I am at-risk of having one or more utilities at my home shut off because I can’t pay my bills (for example, phone, water, electricity, heat, or natural gas).(Required)

3.) Clothing

I am at-risk of not having appropriate clothes to wear for my college classes or work.(Required)

4.) Transportation

I am at-risk of not having safe or reliable transportation to get to and from the NC State campus or work.(Required)

5.) Food

I am at-risk of not having enough nutritious food for myself or people in my household to eat today, tomorrow, or this weekend.(Required)

6.) Childcare

I am at-risk of not having affordable childcare, which I need to attend my NC State classes (on campus or online) or to go to work.(Required)

7.) Medical Care

I am at-risk of not having access to affordable medical care or health insurance for myself or my family.(Required)

8.) Behavioral Health

I am at-risk of not being able to manage my moods.(Required)

9.) Computer & Internet

I am at-risk of not having a reliable computer or internet access to attend my classes and complete my assignments on time.(Required)

10.) Financial Wellness

I am at-risk of not having enough money to pay all of my expenses to attend NC State.(Required)
