Subject:	ACOTE Site Visit Decision for North Central State College

Dear Mr. Chapman, 
At its August 3-6, 2017 meeting, ACOTE reviewed the Report of On-Site Evaluation (ROSE) 
regarding the Occupational Therapy Assistant Program at North Central State College, 
Mansfield, Ohio, and voted to Grant a Status of Accreditation for a period of 7 years. The Report 
of the Accreditation Council (RAC) is viewable at: https://acote.aota.org/programs/2449/report.
A Plan of Correction addressing the areas of noncompliance cited below is due on or before 
October 20, 2017. The report must be submitted through e-Accreditation (https://acote.aota.org) 
using the "POC/PR" tab under "Report." For each cited area, provide a description of the plan for 
bringing the program into compliance with the Standard, a projected time line, a description of 
the documentation to be submitted to demonstrate compliance, and progress on actions (if any) 
implemented to date. Please ensure that you prepare your report in sufficient time to allow for 
review and approval by your dean/administrator prior to the due date.  
 
The plan must include a schedule for correcting the cited areas of noncompliance within the 
following time limit:  
- 12 months if the program is less than 1 year in length;  
- 18 months if the program is at least 1 year in length, but less than 2 years in length; or  
- 2 years if the program is at least 2 years in length.  
 
Please ensure that you are using planned report submission dates that correspond to the ACOTE 
due dates for upcoming meetings: October 20th (December ACOTE meeting), February 20th 
(April ACOTE meeting), or June 20th (August ACOTE meeting). To avoid the potential for 
Probationary Accreditation status, it is strongly recommended that the program plan to correct all 
cited areas at least 4 months prior to the end of the allowable time limit.  
 
Instructions for response to the noncompliant Standards:  
• After you log in to ACOTE online https://acote.aota.org, click on the POC/PR tab under the 
"Reports" Tab. Click on the first noncompliant Standard.  
• Scroll down – ACOTE's concern will be displayed. There will be a box for your narrative 
response/timelines, etc.  
• You may also upload additional Supporting Material by clicking on "Upload More Files."  
• When your response is complete, set your response status as "Ready for Submission." Note that 
all noncompliant Standards must be set to that status before you may submit your response.  
• Click on the POC/PR tab under the "Home" tab to submit your report to your dean for 
approval.  
• The dean will receive an e-mail notification to log into ACOTE online https://acote.aota.org to 
approve and submit the report to ACOTE.

The program's Interim Report will be due in spring 2020 and the next on-site evaluation will be 
scheduled within academic year 2023/2024.  
 
Additional Comment: 
Any institution or program that elects to make a public disclosure of the results of an ACOTE 
accreditation visit must accurately disclose:  
- Statements from the final report, the Report of the Accreditation Council - not the Evaluators' 
Report of On Site Evaluation.  
- Complete information regarding ACOTE's findings (i.e., strengths, suggestions, areas of 
noncompliance, and ACOTE's final accreditation action). Strengths cited in the report may not 
be published without also publishing any cited suggestions or areas of noncompliance.
